Charges are as follows. Please call for hourly base rate ("O.R. Time.")
Not applicable to sub-contracts.
"O.R. Time"—$X/hr. (Time starts 30 minutes prior to the
first case of the day.)
Mileage Allowance—(IRS rate). Distance taken as either estimated
or actual mileage driven.
Insurance Surcharge: A daily rate of $120 to cover insurance premiums.
Overnight accommodations and transportation—by arrangement.
Assigned call duty—by prior arrangement
Daily minimum—8 hours "O.R. Time" or equivalent (5
hour minimum within a 60 mile commute if contracted directly.)
Expenses incurred in obtaining and maintaining hospital privileges
and/or licensure (to be paid directly by the anesthesia group, hospital
or contracting party).
A two week notice cancellation guarantee is required.
I prefer to see the hospital and operating rooms prior to any time commitment. Pulse oximeters and CO2 monitors are required for every case. Insurance requirements limits the number of supervised rooms to two, and I am unable to supervise a room while working in another (except for breaks.) Scheduling priority may be granted to my most active accounts.
