Contractual
Charges are as follows. Please call for hourly base rate ("O.R. Time.")
Not applicable to sub-contracts.
"O.R. Time"—$X/hr. (Time starts 30 minutes prior to the
first case of the day.)
Mileage Allowance—(IRS rate). Distance taken as either estimated
or actual mileage driven.
Insurance Surcharge: A daily rate of $120 to cover insurance premiums.
Overnight accommodations and transportation—by arrangement.
Assigned call duty—by prior arrangement
Daily minimum—8 hours "O.R. Time" or equivalent (5
hour minimum within a 60 mile commute if contracted directly.)
Expenses incurred in obtaining and maintaining hospital privileges
and/or licensure (to be paid directly by the anesthesia group, hospital
or contracting party).
A two week notice cancellation guarantee is required.
I prefer to see the hospital and operating rooms prior to any time commitment. Insurance requirements limits the number of supervised rooms to four, and I am unable to supervise a room while working in another (except for breaks.) Scheduling priority may be granted to my most active accounts.
